How ClearProtection PPO plans work: 

ClearProtection offers affordable coverage with immediate benefits for two doctors' visits. It also covers preventive care and
childhood immunizations, with deductible waived, after the member is on the plan for six months. Here is some information
to help you clearly understand this great plan. 

ClearProtection deductibles and how they work

There are three plan options: $1,000, $3,300 and $5,000. Each plan has two deductibles: one for inpatient, outpatient surgical
and emergency room services; and one for outpatient professional services.

Deductible for Inpatient, Outpatient Surgical and Emergency Room Services ($1,000, $3,300 or $5,000)

• The member pays 100% of the negotiated rate until the deductible is met
• Once the deductible is met, member pays 40% coinsurance on covered services until the out-of-pocket maximum is met
• Once the out-of-pocket maximum is met, member pays 0% coinsurance on covered services for the remainder of the year

Deductible for Outpatient professional services ($4,500, $6,800 or $8,500)

• The member pays 100% of the negotiated rate until the deductible is met
• This deductible equals the total out-of-pocket maximum, so once this deductible is met,
  member pays 0% coinsurance on covered services for the remainder of the year.

Members can satisfy the total annual out-of-pocket maximum in the following ways:

• Meet the inpatient deductible plus coinsurance
• Meet the outpatient professional services deductible (which equals the total out-of-pocket maximum)
• Any combination of the above

The total out-of-pocket maximum equals:

• $4,500 (for $1,000 plan)
• $6,800 (for $3,300 plan)
• $8,500 (for $5,000 plan)
